JOB TITLE: Case Manager

REPORTS TO: Program Supervisor, CPS Prevention Initiative

POSITION SUMMARY: Family Focus is committed to ensuring our centers provide exemplary services with innovative programs and that family support principles are integrated into every aspect of the work. This position plays a central role in fulfilling those two commitments. The Case Manager/Counselor provides support or case management services to 25-45 families participating in the Prevention Initiative Program. She/he also facilitates two ongoing parenting groups and will supervise additional on the request of the Senior Program Manager. This position will primarily be responsible for outreach within the community to enroll participants in the Prevention Initiative program.
To provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual Participants needs. The overall goal of the position is to enhance the quality of participant’s IFSP management, to promote continuity of care, utilization case staffing reviews, and referral to external community services as needed. Has accountability for the care, coordination and termination process for all children aging out of the program. This position is accountable to the Program Supervisor.
- Maintain confidentiality of records relating to participants' program involvement.
- Encourage participants to express their feelings and discuss what is happening in their lives, and help them to develop insight into themselves and their relationships.
- Guide participants in the development of skills and strategies for increasing their ability to access resources.
- Prepare and maintain all required participant records and reports for HV and group services.
- Engage participants, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making life changes with the goal to increase self-sufficiency.
- Collect information about clients through intake, assessments, observation, and individual life history.
- Act as participants’ advocates to coordinate required services or to resolve emergency problems in crisis situations.
- Develop and implement IFSP based on experience and knowledge in family support.
- Collaborate with other staff members to ensure adequate case coordination for all families.
- Recruit and encourage current participants to attend weekly parenting groups.
- Refer participants or family members to community resources such as but not limited to the WIC program as necessary.
- Evaluate the effectiveness of parenting group programs and participants' progress in resolving identified problems and moving towards defined objectives.
- Plan, organize and lead groups, work study, recreation and social activities for participants based on individual identified goals.
- Modify goals or activities and approaches as needed to comply with changes in participants' status.
- Ensure that the trauma informed perspective is integrated and learn about new developments in the zero to three field by reading professional literature, attending courses and seminars, and establishing and maintaining contact with other social service agencies.
- Discuss and develop with individual participants their life plans for life after program involvement.
- Plan and implement play group for children zero to three.
- Meets regularly with PI Coordinator to discuss and review cases.
- Maintain required screenings, update immunizations, educational status, WIC appointments, and family planning for participants and their children.
- Attends and participants in all meeting, conference, in-service training required by Family Focus.
- Facilitate groups at center and collaborating organizations as needed.
- Participate in all center wide trainings, activities, and meetings.
- Work in a team environment.
- Work in other components/capacities as deem necessary by the Center Director to accomplish Family Focus’ Mission.
- Participate in fundraising activities.
- Master’s degree in Human Services, Social Work, Child Development or related field and 2 years of experience working in a strength-based environment with families, children and youth.
- Knowledge and skills in working with families in crisis or trauma exposed situations
- Strong collaborative skills, excellent follow through and excellent relationship building skills is a must.
- Excellent organizational skills.
- Group facilitation expertise.
- Competency in understanding and working with diverse culture and populations.
- Experience in child development, early childhood education, social work, or related field.
- Demonstrated ability to communicate clearly both verbally and in writing.
- Knowledge and understanding of community.
- Requires extensive local travel and some evening and weekend hours
- Must have access to reliable and efficient transportation that will allow for the completion of caseload responsibility
